Posted prices. Paid at the visit.

Your first visit is a $195 evaluation. After that, pay per visit or prepay a pack and save. Everyone pays the same price, and every visit comes with a superbill for your insurance.

Same PT, every visit

Hands-on work and real exercise, every hour.

Each visit mixes manual therapy with exercise you'll actually do at home. Your PT updates your program in the app before you leave, so you're never guessing what to do next.

The HSA figure is a rough guide: federal income tax at your bracket plus Arizona's 2.5% flat income tax, on money you'd otherwise pay with after-tax dollars. Ask your tax preparer how it applies to you.

Your superbill and your insurance

Out of network, on purpose.

We don't bill insurance, so your plan never decides how long your visit is or which exercises you get. You pay here, then file the superbill with your plan as an out-of-network claim.

We give you

  • A superbill after every visit
  • Diagnosis and CPT codes on it
  • Our NPI and your PT's Arizona license number
  • Visit notes if your plan asks
  • Receipts for your HSA or FSA
  • Progress notes for your surgeon or doctor
You file, we help

Your plan decides

  • Whether it covers out-of-network PT
  • Your out-of-network deductible
  • How much comes back to you
  • Whether it wants a referral first
  • How many visits a year it allows
  • How long a claim takes

Coach a team or run a running club?

Return-to-sport testing and injury screens for teams and clubs, priced per athlete, with one invoice. Results go to each athlete, not the coach, unless they say so.

Billing questions

The fine print, in big print.

If something here doesn't answer it, call or text the clinic. A person answers.

When do I pay?

At the end of each visit, by card, HSA or FSA card. Packs are paid at your second visit, once you and your PT have agreed on a plan, so you never buy a pack before your evaluation.

What if I need to cancel?

Move or cancel with 24 hours notice and there's no charge. A late cancel or no-show is $75, and we waive the first one.

What if I don't use my whole pack?

We refund the visits you didn't use, minus the pack savings on the ones you did. Packs are good for 6 or 12 months from the day you buy them.

How does Maintenance work?

Once you've finished a plan of care with us, Maintenance is $160 a month for one 55-minute visit plus texting with your PT. Extra visits that month are $155. A visit you skip doesn't roll over. Cancel with 30 days notice.

I'm on Medicare. What are my options?

We aren't enrolled in Medicare, and physical therapists can't opt out of Medicare and sign private contracts the way some doctors can. That means we can't treat Medicare patients for physical therapy Medicare would cover, even for cash. Call us and we'll point you to a Medicare clinic nearby.
